CLIENT INFORMATION

In case of an emergency, an answering service is available after hours (724-696-9600), as well as on-call staff.

Please be advised that early morning medication hours prior to 07:00a.m. are restricted to those who have employment or other early morning commitments. Please inform your counselor if you need to arrive before 07:00a.m.

CLIENT BILL OF RIGHTS

As a client of RHJ Medical Center your rights include the right to:

Service without discrimination.
 
Information pertaining to policies and procedures of the center.
 
Information about the staff's professional qualifications.
 
Payment and fee information.
 
Discussion of any concerns regarding your treatment.
 
Client confidentiality.
 
Treatment with regard to your civil rights.
 
Request copies of any and all information released to others.
 
Information and referrals to community service organizations.
 
Request to view your client record.
 
Appeal denial of access to your clinical records.
 
Request correction of any information in your client chart.
 
Submit a rebuttal of information contained in your client records.
 
Request a review of discharge/termination criteria.
 
Request review of involuntary discharge criteria.

CLIENT INDIVIDUAL RESPONSIBILITIES IN TREATMENT

It is your responsibility to provide, to the best of your knowledge accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to your behavioral and physical health.
 
You must report perceived risks and unexpected changes in your condition.
 
You must ask questions when you do not understand your care, treatment, and services or what is expected of you.
 .
You must follow instructions for care, treatment and service plans developed and express any concerns about your ability to follow proposed care plan for treatment, care, and services when they occur.
 
You are responsible to accept the consequences for outcomes resulting from a failure to follow the care, treatment and service plan.
 
You are responsible to follow this organizations rules and regulations.
 
You are responsible to show respect and consideration for other clients and staff as well as the facility and its property.
